Prior to being evaluated or treated at The Minnesota Shubert Performing Arts & Education Center, you must have a signed consent form on file. If you have not already filled out a consent form, please print out the consent form below, sign, date, and present the form at your initial evaluation appointment. Please note that if you are a minor, a parent or guardian must sign and date the form prior to evaluation and treatment.
