Free Clinic for Dancers
Minnesota Dance Medicine offers free consultations for dancers at the The Cowles Center for Dance & the Performing Arts.
The clinic is free for dancers of all age and experience.
Since research is essential for furthering information in any field, MDMF is committed to conducting clinical dance medicine research. Our goals are to expand our knowledge and share our conclusions in a usable way with the dance community. Prior to being evaluated or treated, you may be asked to fill out a dancer research survey.
The clinic is typically open one day during the first week of every month by appointment. Other dates are available by request.
- Friday, May 4th, 2018, 2:00-4:00pm Ashley Takekawa, PT and Jodi Zupancich, PTA (Unique screening day. Time spent with both Ashley and Jodi. Be aware that there may be more than one dancer being treated at a time during this screening event).
- Saturday, June 9th, 2018, 9am-12pm Maryann Johnson, PT
We have a suggested donation of $40 for a clinical assessment with a dance medicine provider. Donations can also be made through Razoo
Professional Dancer Free Clinic Dates
Time is set aside for professional dancers of all genres. You will have 30min screening time with a PT on select dates. Schedule is posted and available for booking through Visibook
- Most professional time are listed on Tuesday or Thursday during the day but other times are available by request.
Consent form & Privacy notice
Prior to being evaluated or treated at The Cowles Center for Dance & the Performing Arts, you must have a signed consent form and privacy notice acknowledgement form on file. Please note that if you are a minor, a parent or guardian must sign and date these forms prior to evaluation and treatment. A copy of our current privacy notice can be found here. You can print off the Consent Form and Privacy Notice Acknowledgement Form to complete prior to your appointment.
