Free Clinic for Dancers
Minnesota Dance Medicine offers free consultations for dancers at the The Cowles Center for Dance & the Performing Arts.
The clinic is free for dancers of all ages and experience.
Since research is essential for furthering information in any field, MDMF is committed to conducting clinical dance medicine research. Our goals are to expand our knowledge and share our conclusions in a usable way with the dance community. Prior to being evaluated or treated, you must fill out a consent form and a dancer research survey.
The clinic is typically open one day during the first week of every month by appointment. Available dates are below.

We have a suggested donation of $40 for a clinical assessment with a PT. Donations can also be made through Razoo.

Consent form, privacy notice & dance research survey
Prior to being evaluated or treated at The Cowles Center for Dance & the Performing Arts, you must have a signed consent form and privacy notice acknowledgement form on file. Please note that if you are a minor, a parent or guardian must sign and date these forms prior to evaluation and treatment. A copy of our current privacy notice can be found here. You can print off the Consent Form and Privacy Notice Acknowledgement Form to complete prior to your appointment.
We also require you to have completed a Minnesota Dance Medicine Foundation survey completed to assist us in our research efforts. This information will be provided to you when you schedule an appointment.
